Surgical Treatment of Surgical Neck Fractures of the Humerus in Adults



Abstract
Most proximal humerus fractures respond satisfactorily to conservative treatment. But open reduction & internal fixation or closed reduction & external fixation are recommended in three-part fractures and unreducible two-part fractures. Thirteen
cases
were treated surgically;7 cases with plate and screws, 3 cases with closed reduction and external fixation adn 3 cases with tension band wiring. These three groups were analyzed both clinically and radiologically and the results were obtained as
follows.
1. The results were analyzed according to Neer's criteria, six cases were good, five cases fair and two cases poor.
2. Plate and screw group had a tendencyofanguation at the fracture site. Closed reduction and external fixation was done in old age above 60 and the results wre poor.
3. Complications wee joint contracture and malunion etc, almost was developed in external fixation group. There was no complicaton in tension band wiring group.
4. Tension band wiring technique is a effective, simple and rigid fixation method.
